About the collection
My name is Graham Bellamy. I worked in a small print shop that made ordinary office forms for clinics, schools, warehouses, service companies, and a few municipal departments. Most of the work was not glamorous: pads, duplicate sets, memo blanks, inspection slips, and cover sheets.
I kept unused samples because they show how offices organized work before every field lived inside software. A form is a small argument about what needs to be written down and where the hand is supposed to fit.
What I collect
Blank or generic forms, sample pads, printer proofs, paper-stock notes, obsolete templates, received-stamp examples, and damaged duplicate copies. I avoid private records and do not use real institutional branding for sample scans shown here.
