Maintenance checklists
Maintenance forms take more abuse than memo forms. They ride on clipboards, sit near oil, and get filled out by people standing up.
Equipment checklist
Three-part set: white, canary, pink. The remarks box is large enough for one useful sentence.
Shop-floor copy
Checkboxes are wide enough for a blunt pencil. That was probably intentional.
Initials field
The initials box is small, but it copies well through the yellow sheet because it has a heavy rule.
Checklist habits
The printed checklist decides what is easy to record. If the "remarks" box is too small, people write in the margin. If the date box is cramped, the year disappears first.
On multipart sets the pink copy is usually the worst. Light pencil marks can vanish entirely by the third sheet.
