Incident forms
These are generic blanks, not records of actual incidents. I keep them because the layout shows what an office expected a report to contain before the facts existed.
IR blank, revision 3/91
8.5 x 11 in. The location field is generous, but the action-taken box is shorter than the description box.
Signature box version
Two signature boxes aligned at the bottom. The right box usually collected a received stamp in actual use.
Pad sample
Top glue residue visible on the original. Pads of 50 were cheaper than loose sets but curled more in storage.
Notes
Incident blanks often leave too little room for names and too much room for labels. The form looks orderly when blank. Filled out, the writing usually escapes the boxes.
